中青年晚期肺癌患者疾病不确定感的潜在剖面分析

摘要

目的探索中青年晚期肺癌患者疾病不确定感的潜在类别特征，为精准化心理干预提供理论依据。方法选取2024年4月至2024年9月广州市某三级甲等综合医院收治的211例中青年晚期肺癌患者为研究对象。采用中文版疾病不确定感量表（MUIS-A）调查，通过潜在剖面分析（LPA）探索疾病不确定感的潜在类别结构，并比较LPA结果与K-means聚类结果的分类一致性。结果中青年晚期肺癌患者疾病不确定感均分为（53.30±9.11）分，其中不可预测维度条目均分最高；LPA结果显示，1类模型成功收敛，2~6类模型均因起始值退化为单类而未能产生有效解，提示模型无法识别出多个潜在类别；K-means强制聚类的3类结构与LPA结果无一致性（Kappa=0），强制分类缺乏统计学依据。结论在本研究条件下，中青年晚期肺癌患者疾病不确定感未发现明显的潜在类别区分，临床可将其视为连续变量评估，并重点聚焦不可预测维度开展针对性认知干预。
